The state of Montana requires an additional dose of the Tdap (tetanus, diphtheria, and pertussis) vaccine prior to entering the seventh grade.
Appointments for immunizations can be made at several health care facilities, including at the Laurel Family Medicine Clinic (406-628-6311), Billings Clinic Downtown Pediatrics (406-238-5097), Billings Clinic West Pediatrics (406-238-5800), the Children’s Clinic West End or Downtown (406-281-8700), and the RiverStone Health Immunization Clinic (406-247-3382).
All medical and religious exemption forms are also due prior to the first day of school. Any students without proper documentation will not be allowed to attend until their immunizations and/or documents are current. These exemption forms were revised in 2021, so for any new exemptions, please ensure you use the most up-to-date form, and return forms before the start of the school year.
Vaccination records or exemption forms may be brought to middle school secretaries, faxed to the middle school, emailed (lpsnurses@laurel.k12.mt.us), or returned to the district administration office (410 Colorado Ave, Laurel, MT 59044) in order for your student to begin their seventh-grade school year.
See the Montana Department of Health and Human Services website for more information on immunization requirements at https://dphhs.mt.gov/publichealth/Immunization/childcareandschoolresources.
