Vaccination records or exemption forms should be returned to the district administration office (410 Colorado Ave, Laurel, MT 59044) in order for your student to begin each school year. Records or forms can also be turned in to your child’s school secretary or school nurse.

All medical and religious exemption forms are also due prior to the first day of school and must be renewed and signed annually. Any students without proper documentation will not be allowed to attend until their immunizations and/or documents are current. Medical and religious exemption forms were revised in 2021, so please ensure your forms are up-to-date before the start of the school year.
